Applicants shall prepare a clear scanned copy of the following documents before submitting the online college application for Southern Luzon State University (all campuses):

Must have been taken within the last six months

Must be clear, forward-facing the camera lens, and showing the full face

Only your face and shoulder should be included in the photo

Show natural skin tone

Eyeglasses and colored contact lenses should NOT be worn

Ears should be visible, if possible.

Avoid thick or dramatic make-up. It must reflect your look on the day of the exam, such as with or without a beard.

Must be 2" x 2", colored, with a white background

The digital photo must NOT be a scanned photo of any existing valid ID

The file size must be less than or equal to 2 MB

Must be in JPG file format

For Grade 12 Students expected to graduate at the end of the Academic Year:

• Grade 11 Report Card duly signed by the principal and class adviser

For Senior High School Graduates:

• Grade 12 Report Card duly signed by the principal and class adviser

For Transferees and Returnees:

• Transcript of Records (TOR)/Copy of Grades
• Must have a GWA of 2.25 or better
• No failing and incomplete grades

For ALS Graduates:

• Certificate of Rating indicating "eligible for college admission"

a. For 4Ps members:

• 4Ps Certification from MSWD/DSWD indicating the Household Number and name of the applicants as the beneficiary

b. For IPs Member "Mga Katutubo":

• Certification from the National Commission on Indigenous People (NCIP)

c. For Enhancement of the Comprehensive Local Integration Program (E-CLIP) Beneficiary/ Payapa at Masaganang Pamayanan- Peaceful and Resilient Communities (PAMANA) Beneficiaries:

• Certification from DILG/ Office of the Presidential Adviser on the Peace Process (OPAPP)

d. For Parents employed in Government or Other Agencies:

• BIR Form 2316 (2023)
• Certification from the Human Resource Office indicating its salary and other compensation-related details

e. For Parents working abroad:

• Overseas employment contract

f. For Parents as business owners:

• BIR Form 1701A

g. For With Deceased parents:

• Death Certificate

h. For Solo Parent:

• Solo Parent Certificate from MSWD/DSWD
• Notarized Sworn Statement indicating their current status (single, solo-parent, separated, etc.) and the amount and source of income.

i. For Not classified from above (Unemployed, Housewife/ Househusband, no fixed income, guardianship, self-supporting student and skilled workers):

• Notarized Sworn Statement (indicating the name of Parents/Guardians, and the amount and source of income).

• Certificate of Residency from the Barangay
• Certification of Geographically Isolated and Disadvantage Areas (GIDA) from LGUs (if applicable)

• For students who were delegates or participants in any culture, art, or athletic competitions, they must provide an official written document that they reached the regional level (or higher), specifically any of the following:
• Certification from the current school, signed by the principal & coach that he/she is one of the official candidates (indicate which specific sports or culture and art activities belonged and the highest level reached, for example: Regional, National, International, etc.).
• Certification from the municipality or any institution (Private or Public) that he/she is one of the official candidates (indicate which specific sports or culture and art activities belonged and the highest level reached, for example: Regional, National, International, etc.).
• Written endorsement from the Head of the Sports and Cultural Program from your academic institutions.

Step 1: Visit the SLSU Student Admission Office FB Page (@SLSUadmission) or www.slsu.edu.ph for the announcement of SLSU College Application AY 2025-2026.

Step 2: Prepare the scanned copies of the application requirements.

Step 3: Application may be online or you may visit the SLSU Student Admission Office (MIS-Building) for a walk-in application.

Step 4: Prepare a valid and active email address.

Step 5: Accomplish the ONLINE COLLEGE APPLICATION FORM to be posted on the official SLSU website and Facebook Pages

Step 6: Examination permit and other admission-related updates will be sent through your provided email address. Check your inbox regularly.

Step 7: Arrive at your scheduled examination date. Make sure to be available an hour before your time schedule, bring at least two (2) pencils, a printed examination permit, and wear a white t-shirt, maong pants, and comfortable shoes (refrain from wearing crop tops, shorts, slippers, and other revealing/distracting clothes). Phones and other gadgets are also not allowed on the examination site.
